Kryptogene organisierende Pneumonie (COP)
Organisierende Pneumonie = Histopathologischer Befund
beschrieben durch:
intraalveoläres Granulationsgewebe (bestehend aus Fibroblasten und Myofibroblasten)
und
Bindegewebe (bestehend v.a. aus Kollagen)
bei fehlender Ätiologie:
Kryptogene organisierende Pneumonie
Inzidenz: 1,97/100 000

Kryptogene organisierende Pneumonie (COP)
PATHOGENESE:
1. entzündlich bedingte Exsudate
2. organisieren sich zu herdförmig
begrenzten alveolären Zell-
Clustern
3. intraalveoläre Fibrosierung
Sekundär werden die Bronchiolen
miteinbezogen (durch okkludierende
Bindegewebsproliferationen)

IPFCOP
- reversibel- höhere apoptotische Aktivität
- Fas und TNF-α
- M30 (Caspase gespaltene Cytokeratin-18 Fragmente)
- Reepithealisierung
- Expression von ING4(Angiogeneseinhibitor)
- VEGF
- MMP2 + TIMP (tissue inhibitor of MP)
(sekretiert von Fibroblasten und Epithelzellen)
normale Wundheilung
- irreversibel- geringere apoptotische Aktivität
- Fas und TNF-α
- M30 (Caspase gespaltene Cytokeratin-18 Fragmente)
- Reepithealisierung
- Expression v. Surfactant Protein-A(immunologische Funktion)
- VEGF
- MMP9(sekretiert von Entzündungszellen)
überschießende Wundheilung

KLINIK:
sehr unspezifisch
- 50 – 60 Jahre- m : w = 1 : 1- häufiger Nichtraucher und Ex-Raucher betroffen (v.a. bei Frauen)- leichtes Fieber, Husten, Gewichtsverlust, leichte Dyspnoe- Dauer: weniger als 3 Monate- Auskultation: spärliches Knisterrasseln über den betroffenen Arealen- LuFu: moderate restriktive Ventilationsstörung- BAL: Lymphozyten (20-40%), Neutrophile (≈10%), Eosinophile (≈5%)
erniedrigter CD4/CD8-Quotient
