The 2-year failure free survival rate was 44.8% (95%CI: 32.5-56.4%) after BEP and 58.2% (95%CI: 48.0-71.9) after HD-CT

• Data on 1594 male patients treated with first line cisplatin/etoposide based chemotherapy between 1990 and 2008 were collected from 38 centers in 14 countries in Europe, Canada and the USA
• All patients received their first salvage therapy

• Five prognostic groups were identified based on seven clinical variables
- histology- primary tumor site- respone to first-line chemotherapy- progression-free interval- levels of AFP and HCG- liver, brain, bone metastases

Schlussfolgerungen zur Hochdosistherapie (2) 2nd line / 1. Rezidiv:
kurative Chance liegt bei circa 50 %, stark abhängig von Prognosefaktorenkein randomisierter Vergleich zugunsten von HD –CTX verfügbargrößte Metaanalyse auf Basis individueller Daten zeigt Vorteil von 10 % im LZ - Überleben mit HD CTX in allen Prognosegruppenvereinzelt profitieren auch ciplatin-refraktäre Patientenlaufende Registerstudie zur Salvagetherapie in Deutschlandweltweite Studie mit Randomisierung gegen TIP geplant („TIGER“)

Schlussfolgerungen zur Hochdosistherapie (3)
3rd line / mind. 2. Rezidiv:
keine kurative Option mit konventionell dosierter CTX mehr,HD CTX ist „Standard“ (wenn Pat vorher nur SD-CTX hatten) HD-CTX erzielt aber nur noch LZ-ÜLR von 15- bis max. 20 %
Therapie-induzierte Leukämien
Chemotherapie ist mit Risiko der Entwicklung sekundärer Leukämien assoziiert
höchstes Risiko besteht in den ersten 5-10 Jahren nach Therapie,kein erhöhtes Risiko nehr nach 10 Jahren
therapie-assoziierte Leukämien besitzen schlechte Prognose
die kumulative Dosis von Etoposid und das Applikations-schedule beinflussen das Risiko für therapieassoziierte Leukämien

Geringes Risiko nach first-line HD-Etoposid (1/323 = 0.3%)
