Is the risk of CV events increased for a generic version of slow release metoprolol vs. the originator product ?
Bremer Institut für Präventionsforschung und Sozialmedizin
Does the generic beta-blocker B carry a higher risk for hospitalizationthan the originator beta-blocker A ? o due to MI, stroke, hypertensive crisis (index events) o due to any (other) CVD-events o due to any cause
– Detectable RR=1.23 (α=0.05; 1-β≥0.80)
Potential confounders: o age, sex, place of residence, SHI, duration of insurance o smoking (indirect assessment by COPD medication) o comorbid conditions (e.g. previous MI) o co-medication o dosage o previous treatments/ treatment changes

• Study design – Historical cohort study – Subjects: members of 3 SHIs (Bremen/Northern Germany)
– Period: o Study period 2002 to 2003 o Membership 2000 to 2003 (previous comorbid conditions)
– Inclusion criteria: o Prescription of beta-blocker A or B between 2002-2003 o No indication of congestive heart failure
(based on prescriptions according to NYHA (New York Heart Association) functional class III and IV)

• Signalgenerierung
• große Population zur Erkennung seltener Arzneimittelrisiken – Hospitalisierungen zur Untersuchung schwerer Arzneimittelrisiken – lange Beobachtungsdauer zur Erkennung von Langzeittoxizität – Rapid – Response Ansatz möglich
• Monitoring von Anwendung/Sicherheit neuer Arzneimittel
• rel. kostengünstige Durchführung von Studien
