„One of the primary function of the cellular immunity is to protect from neoplastic disease thereby maintainig tissue homeostasis on complex multicellular organisms“

Ansprechen vorhersagen? Resistenzmechanismen 31
Hyperprogressive Disease: a new pattern of progression in pts. treated with anti-PD-1/PD-L1
131 patients treated with anti-PD-1/PD-L1 in phase I clinical trials were evaluable for their tumor growth rate (TGR) before treatment ("REFERENCE period"; REF) and upon treatment ("EXPERIMENTAL period"; EXP)
Hyperprogressive disease (HPD) was defined as disease progression by RECIST criteria with a 2-fold increase in the TGR EXP versus REF.
12 patients (9%) with an HPD pattern HPD:
not associated with advanced disease equally observed with PD-1/PD-L1 blockers Observe across all tumor types. associated with an older age (>65 y) associated with LDH increase associated with worse overall survival.

Terminology for different resistance mechanisms to immunotherapy
Primary resistance: A clinical scenario where a cancer does
not respond to an immunotherapy strategy
Adaptive immune resistance A mechanism of resistance where a
cancer is recognized by the immune system but it protects itself by adapting to the immune attack
Acquired resistance A clinical scenario in which a cancer
initially responded to immunotherapy but after a period of time it relapsed and progressed

Whole-exome sequencing in 4 melanoma pts. with acquired resistance to PD-1 blockade
In 2/4 patients resistance-associated loss of-function mutations in the genes encoding interferon-receptor–associated Janus kinase 1 (JAK1) or Janus kinase 2
A truncating mutation in the gene encoding the antigen-presenting protein β-2-microglobulin was identified in a 3rd patient
Conclusion: acquired resistance to PD-1 blockade in melanoma pts. associated with defects in the pathways involved in interferon-receptor signaling and antigen presentation.
