Indication

Einleitung Introduction

Für die Versorgung von Tibiakopf- und pro-ximale Unterschenkelfrakturen standenbisher Platten mit Standardschrauben,Doppelplatten-Osteosynthesen, Fixateur-Versorgung und intramedulläre Verfah-ren sowie Kombinationen der genanntenVerfahren zur Verfügung.Nachteile: je nach Verfahren besteht einehohe Weichteilbelastung, eine hohe Kno-chenbelastung durch Denudierung, Gefahrdes Implantatversagens, axiale Instabilitätmit Sekundärdislokationsgefahr. Dies kannzu Pseudarthrosenbildung, Implantatlocke-rung und -bruch und zu Wundheilungs-Störungen mit nachfolgender Infektion

führen gefolgt von notwendigem Verfah-renswechsel.Für eine weichteilschonendere teilweiseminimalinvasive und dennoch repositions-verlustfreie Osteosynthese steht nun diewinkelstabile, anatomisch geformte, latera-le Tibiakopfplatte (4,5 mm) zur Verfügung.Das vorgestellte Implantat ermöglicht einprimär denitives Verfahren mit Weichteil-schonung und Erhalt der Knochendurchblu-tung. Ein unilaterales Vorgehen ist auf Grundder Winkelstabilität möglich. Die Schrau-

benrichtung ist anatomisch optimiert.Das herkömmliche 4,5 mm Standard-Instrumentarium ist kompatibel.

For the management o ractures o the tibial head and

proximal shat, available methods employ plates, double

plates, fxators and intramedullary procedures and

combinations o these.Depending on the procedure, there is high sot tissue

stress, high bone stress due to denuding, risk o implant

ailure and axial instability with the risk o secondary

dislocation. This can lead to the development o a

pseudar throsis, implant loosenin g and racture and

delayed wound healing with subsequent inection,

ollowed by the need or a change o procedure.

The angle-stable, anatomically shaped lateral tibial head

plate (4.5 mm) is now available or a sot tissue-sparing

internal fxation that is as minimally invasive as possible

and nevertheless ree rom loss o reduction.

The implant presented here enables a primary defnitive

procedure to be carried out with sparing o sot tissues

and preservation o bone perusion. A unilateral

procedure is possible because o the angular stability.The screw direction is anatomically optimised. Conven-

tional 4.5 mm standard instruments are compatible.

Nachbehandlung

After-Treatment- Schienenlagerung bis Redonzug (2. Tag)

- Nach Redonzug Krankengymnastik - Bewegungsumfang bzw. Orthese nach

intraartikulärer Begleitverletzung- Entlastung mit erlaubtem Bodenkontakt

nach Frakturform und Begleitverletzungmindestens 6-8 Wochen

- Splinted position until removal o drain (2nd day)

- Physiotherapy ater removal o drain- Range o movement or orthesis ater associated

- Discharge with oor contact permitted depending on

racture type and associated injury at least 6-8 weeks
