Auf den folgenden vier Folien ist die Methodik der Verbrauchsberechnung erläutert, welche dem individuellen Krankenhaus-Antiinfektiva-Report sowie dem Krankenhaus-Vergleichsreport zu „benchmark“-Zwecken zugrundeliegt.

ADKA-if-RKI – Projekt - Berechnung des Verbrauchs I -

Beispiel: Station A verbraucht 15.000 Infusionslösungen Cefuroxim 1,5g im Jahr. Anzahl der Pflegetage im Jahr sind 20.000.

Berechnung:

Anzahl verbrauchter Tagesdosen: = Gesamtgramm Substanz / Tagesdosis in Gramm

= 15.000 x 1,5g / Tagesdosis in Gramm = 22.500 Gramm / Tagesdosis in Gramm Voraussetzung:

Tagesdosis für die einzelne Substanz in Gramm ist festgelegt

DDD (defined daily dose) nach dem ATC/DDD-System der WHO (www.whocc.no)

= angenommene mittlere tägliche Erhaltungsdosis für die

Hauptindikation eines Arzneimittels bei Erwachsenen

häufig „zu“ niedrig (Bspl.: Ceftazidim 4g iv, Cefuroxim 3g iv)

Beispiel einer Verbrauchsdarstellung für das gesamte Krankenhaus für ein Jahr (hier 2010) über 4 Quartale als Anwendungsdichte (RDD/100 sowie DDD/100). Zum Vergleich sind die Verbrauchsergebnisse für RDD (grau) und DDD (weiß) nebeneinander dargestellt. Für einzelne Fachabteilungen erfolgt die Verbrauchsdarstellung jeweils auf einer weiteren Seite (s. Beispielreport im Anschluß an die Präsentation). Separate Angabe von Pflegetagen und Fallzahlen, dadurch ist eine Berechnung der Liegedauer pro Quartal möglich. Durch die zusätzliche Angabe der Anzahl abgegebener Tagesdosen (RDD bzw. DDD) ist eine Berechnung der Tagesdosen pro Fall ebenfalls möglich.

Darstellung des Antiinfektiva-Gesamtverbrauches mit separater Darstellung des Verbrauches für Antimykotika, Virustatika, Tuberkulostatika, antiparasitäre Medikamente und Antibiotika.


Beispielhafte Verbrauchsdarstellung der Antibiotika einer chir. Intensivstation mit separater Darstellung der Betalaktame, der Fluorchinolone, der Glykopeptide, der Aminoglykoside und sonstiger Antibiotika. Die Betalaktame sind unterteilt in 3. Generations-Cephalosporine, Breitspektrum-penicilline, Carbapeneme, 1./2. Generations-Cephalosporine und Aminopenicilline/ Betalaktamaseinhibitor, etc.. Sonstige Antibiotika werden in Makrolide, Tetrazykline, Folatantagonisten und ausgesuchte Einzelsubstanzen unterteilt.


Beispielhafte Verbrauchsdarstellung der Top 15 Antiinfektiva einer internistischen Normalstation nach Substanz und Applikationsart über die Quartale der letzten zwei Jahre, absteigend sortiert nach höchstem Verbrauch (in RDD) im jüngsten Quartal. Dies ermöglicht eine Verlaufsbeobachtung der Platzierung einer Substanz unter den 15 am häufigsten verordneten Antiinfektiva (incl. Antimykotika, etc.).
