1 Rezeptierungen in den USA (Prescriptions)
Es ist in amerikanischen Krankenhäusern durchaus üblich, dass der Student für seinen Patienten – z. B. in der Ambulanz – die Medikamente vorschlägt und diese mit dem Oberarzt bespricht. In diesem Rahmen wird vom Studenten zugleich erwartet, ein (fast) vollständig ausgefülltes Rezept vorzulegen, damit Zeit gespart wird und man sich schnell dem nächsten wartenden Patienten zu-wenden kann.

Zu beachten ist, dass das Rezept für den Apotheker ausgestellt wird. Dieser „übersetzt“ die Kürzel für den Patienten und etikettiert jedes Medikament mit den ärztlichen Anweisungen. Dies sollte den Studenten/Arzt jedoch nicht da-von abhalten, den Patienten über die Einnahme der verordneten Medikamente zu informieren!In jedem Fall sollten auf dem Rezept der Name des Patienten sowie seine Daten eingetragen werden. Das Medikament und die Anweisung zur Einnahme sowie die Menge können auch erst nach Rücksprache mit dem verantwortlichen Arzt eingefügt werden.

Darüber hinaus ist es wichtig zu wissen, dass in den USA oft Medikamente in kleinen Probepackungen an Sozialschwache ausgegeben werden können. Diese werden von der Pharmaindustrie selbst gestift et. Allerdings wird im Gegenzug verlangt, jedes ausgegebene Medikament schrift lich zu dokumentieren. Dies bedeutet weiteren Papierkram, der zumeist von einer Pfl egekraft übernommen wird.Auch in den USA gibt es eine so genannte Positivliste aller Medikamente, deren Wirksamkeit in Studien belegt wurde. Nach dem Wirkstoff ist der Preis das wichtigste Kriterium bei der Auswahl der Medikamente.

4 EKG – Physiologische und pathologische Befunde
Die korrekte Interpretation von EKGs ist sicherlich eines der meist unter-schätzten Defi zite während der klinischen Ausbildung deutscher Mediziner. Dies belegen auch die zahlreichen Veröff entlichungen zum Th ema „EKG lesen leicht gemacht“ oder „Das EKG – wie helfe ich mir selbst?“ o. ä. Dabei können – bei richtiger Vorgehensweise – mit relativ wenig Aufwand schon sehr viele Informationen gewonnen werden, die in Verbindung mit der „Klinik“ eines Pa-tienten häufi g eine Arbeitsdiagnose ermöglichen.Das folgende Kapitel soll das geeignete Vokabular zur Interpretation vorgeben – einschlägige Fachliteratur kann und soll nicht ersetzt werden. (z. B. „Vom EKG zur Diagnose“, Schmitt, Springer Verlag oder „EKG-Kurs für Isabel; Georg Th ieme Verlag, Stuttgart). In strukturierter Reihenfolge werden die Normwerte pathologischen Befunden gegenübergestellt.
4.1 Wie lese ich das EKG?
Bei einer streng chronologischen Vorgehensweise der EKG-Interpretation ist das Risiko, auff ällige (d. h. nicht „normgerechte“) Befunde zu übersehen, ver-hältnismäßig gering.Bei Beachtung der einzelnen „Bausteine“ rate, rhythm, axis, P wave, PR interval, QRS complex, QT interval, ST segment und T waves ist es einfach möglich, die wesentlichen Merkmale eines EKG-Befundes zu erfassen.Kleiner Tipp am Anfang: Zuallererst sollte man sichergehen, dass man das zu dem gewünschten Patienten gehörende EKG befundet – stehen Name, Ge-burtsdatum, Wohnort, Station, o. ä. auf dem EKG und ist so eine eindeutige Zuordnung möglich?Die in nachfolgender Aufstellung verwendeten Abkürzungen fi nden sich auch im Abkürzungsverzeichnis.

4.2 Was noch von Interesse sein könnte...
Der Schnellzähler
Zur einfachen Ermittlung der Herzfrequenz anhand eines EKG gibt es die Schnellzähler-Methode:Einen deutlichen QRS-Komplex (bzw. deutliches R) lokalisieren, von dort vor- oder rückwärts zum nächsten QRS (R) zählen, wobei alle 5 mm (große Käst-chen) wie folgt rückwärts gezählt wird: 300 – 150 – 100 – 75 – 50 – 25.
Offi ziell entspricht bei einem Vorschub von 25 mm/sec ein „großes“ Kästchen 200 msec und ein „kleines“ Kästchen 40 msec.

Die „Six-Second“-Methode
Diese Methode bietet sich bei v. a. bei Arrhythmien an, um zumindest eine durchschnittliche Frequenz errechnen zu können:30 große Kästchen abzählen (= 6 sec; da 0,2 sec (oder 200 msec, s. oben) × 30 = 6 Sekunden. Jetzt die Anzahl der R-R-Intervalle innerhalb dieser sechs Sekunden abzählen und mit 10 multiplizieren (Schläge/Minute).

Lagetypenhexerei
Die Lagebestimmung der Herzachse ist keine Hexerei. Sie ist eigentlich ganz simpel – vorausgesetzt, man hat folgendes Bild vor Augen (Abb. 11):

daraus ergibt sich:• wenn I und aVF (und II) überwiegend positiv, dann NA (normal axis),• wenn I überwiegend negativ, aVF überwiegend positiv, dann RAD (right
axis deviation),• wenn I überwiegend positiv und aVF überwiegend negativ, dann LAD (left
axis deviation).
Niederspannung (Low Voltage)
Low voltage, defi niert als QRS <5 mm in allen peripheren Ableitungen, gilt als Marker für Perikardergüsse. Darüber hinaus können auch hypothyreodism, COPD, ARF, subcutaneous emphysema, PE, VCDs, myocarditis, pericarditis, car-dial amyloidosis, drug-induced (Adriamycin) die Gründe sein.Kleiner Tipp: Gerade in den USA empfi ehlt es sich darüber hinaus das Gewicht des Patienten zu erfragen, da auch eine Adipositas das EKG dämpfen kann. CAVE! Dementsprechend kann bei extrem schlanken Patienten eine myokar-diale Hypertrophie aufgrund der hohen Spannung vorgetäuscht werden!

Infarktlokalisation

Infarkt ohne Q-Wellen-Beteiligung (Non Q Wave Infarction)
Non Q wave infarction hieß früher subendocardial infarction. Es kommt zu ST und T wave changes ohne Ausbildung eines pathologischen Q. Es führt also nicht zu einer transmuralen Schädigung; dementsprechend ist meist auch die Prognose günstiger.
Die Lungenembolie (Pulmonary Embolism)
Die Lungenembolie wird auch als Chamäleon bezeichnet, da die Symptomatik häufi g unspezifi sch ist bzw. die anderer Erkrankungen (Myokardinfarkt, Peri-karditis, Aortendissektion) nachahmt. Am häufi gsten fi ndet sich eine sinus ta-chycardia. Die akute Rechtsherzbelastung zeigt sich häufi g in der Abweichung der Herzachse nach rechts (RAD) und neu auft retenden Blockbildern (RBBB). Seltener, dennoch immer wieder in Prüfungen abgefragt, tritt der SIQIII-Typ auf: Tiefes S in I und pathologisches Q in III, wobei die Briten noch TIII (also SIQIIITIII) hinzufügen, eine T-Negativierung in III.
Stoffwechselstörungen (Metabolic Abnormalities/Disorders)
Stoff wechselstörungen können sich ebenfalls am Herzen manifestieren. Am häufi gsten sind Störungen des Kalium- und Calciumstoff wechsels sowie kar-diogene Eff ekte unter Digitalistherapie.Digoxin eff ect: ST depression und T inversion in V5 und V6 Digoxin toxicity: Auft reten sämtlicher Rhythmusstö-
