CURRENT POSITION (Consultant Haematologist)

From 16th of February 2009 to the present, I hold the post of consultant haematologist (permanent position) at the Department of Haematology and Bone Marrow Transplant (BMT) Unit, San Maurizio Regional Hospital, Bolzano, Italy. This is a centre of excellence for the treatment of benign and malignant Haematological conditions and a BMT centre of excellence for North Italy It serves the Italian Region of South Tyrol/Alto Adige which has a population of 500.000. The centre is accredited by JACIE, ISO 9001, and participates in multicentre clinical studies in collaboration with - GIMEMA: Gruppo Italiano Malattie Ematologiche Maligne dell'Adulto; - NHLCSG: Non Hodgkin Lymphoma Cooperative Study Group - GITMO: Gruppo Italiano Trapianto di Midollo Osseo - GITIL: Gruppo Italiano Terapie Innovative nei Linfomi - DSMM: Deutsche Studiengruppe Multiples Myelom The medical director of the centre is Professor Sergio Cortelazzo (University of Verona). The Department includes another 11 consultant haematologists, 31 nurses and 11 lab-technicians. The ward has 14 beds; the Day Ward has 10 beds. The centre currently performs approximately 50 autologus and 25 allogeneic bone marrow transplantations per year.

CLINICAL DUTIES IN CURRENT POST:

- The diagnosis and management of patients on the in-patient ward and the day ward, undergoing standard chemotherapy regimes and BMT (autologus and allogeneic). The outpatient clinical follow up of patients with acute leukaemias, lymphomas and other haematological conditions attending the day ward. - Prescription of blood products, management of transfusion reactions, reporting of non conformant, reporting of cases of TACO and TRALI, decision making regarding blood provisions before and during allogeneic transplantation (ABO major and minor). - For patients undergoing BMT my duties include preparation of BMT elegibility, BMT work-up, pre-transplant meetings with transfusion team, organisation of peripheral blood stem cells collections, bone marrow harversting etc. The centre is JACIE accredited all clinical procedures are performed to the highest standards and in accordance with accreditation processes. - I participate on the consultant on call haematology rota which also includes medical cover for neurology and gastroenterology wards. - My clinical duties involve multidisciplinary team working and close

collaboration with medical colleagues across other medical specialties.

RESEARCH ACTIVITIES IN CURRENT POST

- I am currently collaborating with Dr Cinzia Vecchiato, responsible clinican for the HLA laboratory (Transfusion Service, Bolzano Hospital) on a joint research project on Autologus Reconstitution post BMT (I have previous experience of using capillary eletrophoresis ABI PRISM, see publication p1 , page 18). This project focuses on HLA and KIR relationships in autologus reconstitution. In particular we are working on a prospective study in severe aplastic anaemia (SAA) approved by the EBMT SAA-WP group. We will apply for a research grant to the South Tyrol Region. The study will be run in collaboration with the University of Innsbruck, Austria and the University of Padova, Italy (Prof Renato Zambello).

- I am leading a research study on microparticles generation and endothelial modulators in essenthial thrombocythosis, polycithemia vera and multiple myeloma in collaboration with the IBTS (Dr William Murphy, Elva Eakins, Ciaran Murphy) the Clinical Chemistry Dept, Onze Lieve Vrouwe Gasthuis, Amsterdam, The Netherlands (Dr Muriel van Schilfgaarde). This project is fully funded by a grant obtained by a Suisse Fundation on Tumors (Prof. Paolo Rebulla, Transfusion Service, Ospedale Maggiore, Milano, Italy).

- I am also working on a clinical study on a large cohort of patients with low risk Essenthial Thrombocythosis analysing full blood count fluctuations in collaboration with other North italian centers (Venice, Padova, Trento, Treviso, Mestre, Udine and the University of Innsbruck, Austria).

TEACHING ACTIVITIES IN CURRENT POST

- I have recently started the accreditation process to become an associated Professor of Haematology in Italy (MIUR) and in Austria at the University of Innsbruck, in agreement with the medical Director of the Haematology Oncology and Internal Medicine Dept Professor Guenther Gastl. I have already given lectures at Innsbruck University.

- Supervision and teaching of medical students (from Medical Universities in Hamburg, München, and Rostock) and junior doctors (from general medical training scheme of Hamburg University) while on medical rotations.

- I participate in the hospital haematology teaching programme for staff.

- Since taking up this post I have been an invited speaker at a number of

academic haematological meetings including – a national academic meeting at Padova University, Innsbruck University (Austria) and an SPR meeting at Verona University.

(Oct 1998-Oct 2002):

In October 1998 I passed the admission exam for entry on to the Specialist Registrar Training Scheme in Haematology at Verona University, Italy under Professors G.Perona and G.Pizzolo. This is a four-year training scheme and that involves annual clinical and written exams and is recognised as equivalent to the Irish SpR training scheme in Haematology under the European Law 93/16 on the 5th April 1993 and EU Directive 2005/36/EC

CLINICAL ROTATIONS IN HAEMATOLOGY

• From 01/10/1998 to 28/02/2001: Specialist Registrar in Haematology at the Haematology Department of Vicenza Hospital “San Bortolo”, under Prof. F.Rodeghiero. I was involved in the care of patients with acute leukaemias and other haematological malignancies. This included both diagnosis (morphological bone marrow reporting and immunophenotyping procedures), and treatment, including the prescribing of chemotherapy. I was mostly involved in the BMT Unit. I gained experience in the clinical management of patients undergoing Allogeneic and Autologus BMT, and associated complications. I also worked in the Day-Ward, where I was responsible for the medical follow-up of BMT patients, as well as the assessment and management of patients with general haematological problems. I also gained experience in coagulation through my attendance at the Coagulation Clinic. This is one of the most prominent centres in Italy for the treatment of Haemophilia patients and research on Von Willebrand’s Disease.

• From 01/03/2001 to 04/03/2002: Clinical Registrar in Haematology, at the Department of Haematology of St. James`s Hospital, Dublin, under the direction of Prof. Shaun McCann, Dr Paul Browne and Dr Eibhlin Conneally. This year of overseas training was recognised as an accredited year of training by my Italian training scheme. I worked mainly in the Day-Ward where the patient cohort consisted primarily of post-BMT patients and other chronic haematological conditions. I was also responsible for the assessment of new patients and the management of haematological emergencies. During this post I also worked in the Haematology Laboratory where my responsibilities included bone marrow and blood film reporting.

• From 05/03/2002 to 01/11/2002: Specialist Registrar in Haematology at the Haematology Department of Vicenza Hospital “San Bortolo”, under Prof. F.Rodeghiero My clinical duties were similar to those outlined above.

• From 4/11/2002 to the 31/12/2003 working in a Specialist Registrar post in Paediatric

Onco-Haematology at “Our Lady’s Hospital for Sick Children”, Dublin, under the supervision of Prof. O.Smith, Dr A O’Marcaigh and Dr C. Mc Mahon. I gained clinical experience in the treatment and management of benign and malignant paediatric-haematological conditions. My main clinical duties in the Dept of Paediatric Onco-haematology included the management of patients in the BMT-Unit, the ward, the outpatient clinic and the Day Ward. I collaborated with the accreditation/Self Assessment team in Oncology and

Haematology, created to provide and facilitate the audit and accreditation process at Our Lady’s Hospital for Sick Children under the direction of Dr Uma Rao.

• From the 01/01/04 to 31/5/04, Senior Registrar in Haematology at the Irish Blood

Transfusion Service, St James’s Hospital, Dublin, under the supervision of the National

Medical Director, Dr. W. Murphy. My duties included giving advice on the appropriate use of blood products, platelet refractoriness, paediatric transfusion, HDN and NAITP, transfusion reactions, provision of HLA matched products; assessment and care of blood and platelet donors; the assessment, care and management of patients during therapeutic aphaeresis. I also participated on the on call rota for Specialist Registrar in Transfusion-Haematology.

• June 2004 – February 2009: PhD Research Fellowship, at the Irish Blood Transfusion

Service and the Paediatric Haematology Department Our Lady’s Children Hospital in Dublin, under the supervision of Prof O.P. Smith (University of Dublin, Trinity College, Ireland), Dr W. Murphy (National Medical Director IBTS) and Dr C. Mc Mahon (OLHSC). My PhD thesis entitled “New insights in Sickle Cell Anaemia Pathophysiology” was completed and accepted by TCD in December 2009 (full description of Ph.D provided in research experience section of CV).

During this position I gained a wide range of clinical experience: medical management of soldiers admitted to the military medical unit (e.g. managing chest infections, meningitis, cardiovascular disease, etc) provision of emergency mountain rescue service with helicopters (eg managing trauma sustained from climbing accidents/skiing), vaccination of soldiers, provision of medical first aid to the military staff). In addition to clinical experience, this position allowed me to gain significant management experience ( e.g. organisational aspects of the medical care provided to military staff and resource management).

MANAGEMENT EXPERIENCES

• Consultant haematologist (current position) and previous clinical posts In my current post as consultant haematologist and also in previous clinical posts, the following management skills and experience form/have formed an essential part of my routine clinical practice: clinical leadership, multidisciplinary team working, conflict resolution, time management, management of financial budgets and clinical resources.

• I have a good understanding of the structure and organisation of both the Irish and Italian Health Care System

• Diploma in Medical Management (2005) I completed a post graduate Diploma im Medical Management - Royal College of Surgeon in Ireland and Institute of Public Administration.

• PhD Thesis preparations (2004 - 2009) During the preparation of my PhD thesis I improved my management and leadership skills. For example, I gained skills in how to optimise resources (laboratories kits, delivery by couriers, travel expenses), to motivate and empower others involved in the research project (lab technicians, phlebotomists, nurses).

• Academic peer review publications During my career I have produced several publications, most of which are interdisciplinary studies with other branches of Medicine outside of Haematology. I have learnt to involve other medical colleagues and health care professionals in common tasks, creating a sense of curiosity and a common vision on projects. I have gained an appreciation of the importance of listening to different opinions and the use of constructive criticism. In order to complete my research projects, I gained important skills in time management, delegation skills, setting and meeting goals, managing and adhering to a research budget, and acting as project leader.

• 1992/1993: Winner of a scholarship sponsored by Wűrzburg University, Germany, During this period I successfully passed the exams in Neuroradiology, Special Pathology, Internal Medicine (part I and II) and General Surgery (part I). I spent most of my time under the direction of Prof. H Müller Hermelink, in the Special Pathology Institute. I was involved in post-mortem examination, biopsy interpretation and diagnosis, immunocytochemistry, histology sample preparation (slides staining, use of microtomy).

• August 1994: Winner of a scholarship sponsored by Kiel Medicine University, North-

Germany, in the Paediatric Department for Premature Newborns (Neufrűegeborene Station).

• 1st MD thesis University of Padova, Italy (1993-1996) "Gene Therapy in Human Medulloblastoma, in Vivo and in Vitro" This thesis was presented and accepted by the Medical University of Padova in 1996. It was carried out under the supervision of Prof. Luigi Zanesco, director of the Dept of Paediatric Oncho-Haematology of Padua University Hospital (the largest centre for Paediatric Onco-Haemathology in Italy). This research focused on the bystander effect of gene therapy in vitro and in vivo (using a mouse model with Di George Syndrome). In particular we were able to identify the presence of connexin 43 in several cell lineages and to enhance its expression using retinoic acid. We also were able to grow medulloblastomas cell lineages in vitro and in vivo and to demonstrate the efficacy of gene therapy using the timidine kinase retroviral model. While conducting this research I gained laboratory experiences in performing Western Blot, PCR, cell culturing, working with laboratory animals, and performing intracranial stereotaxical inocula in nude mice and rats.

• 2nd MD thesis, University of Verona, Italy (1998-2000) Analysis of conventional and high Dose Therapy in a casuistic of 201 cases of Multiple Myeloma This research was completed in the Haematology Dept of Vicenza Hospital, under the supervision of Prof. Francesco Rodeghiero. It was a study on a large cohort of patients with Multiple Myeloma and the outcome results following chemotherapy, single and double autologus-BMT.

• PhD Thesis, University of Dublin, Trinity College, Ireland (2004-2009) Sickle Cell Anaemia. New insights into its pathophysiology

This thesis was presented and accepted (without needing further corrections) at University of Dublin, Trinity College, Ireland in August 2009. The degree was conferred to me in December 2009. Examiners were Prof Hann Ian (Great Hormond Street’s Hospital, London, UK) and Dr Aengus O’ Marcaigh (OLHSC, Dublin). The entire research was completed during a full time research post, shared between the Irish Blood Transfusion Service & the Dept of Paediatric Haematology, Our Lady’s Children Hospital in Dublin, under the supervision of Prof Owen Patrick Smith (University of Dublin, Trinity College) and Dr William G Murphy (National Medical Director IBTS and UCD)

The intent of this project was to provide a clinical and laboratory PhD thesis on Sickle Cell Anaemia in a paediatric population group of African origin. The research focuses on endothelium activation/apoptosis markers during all the stage of sickle cell anaemia. Ethical permission for the study was obtained and written consent signed by all the parents of the children involved in this study. A samples library was created at -80°C.

The main research focus was the study and detection of microparticles. An experimental Flow Cytometry method was designed for this purpose. This research also involved the study of endothelial marker expression (e.g. Endothelin-1, Nitric Oxide, adrenomedullin etc).

One of the main findings of this study included the presence of lower levels of protein C and free protein S in children with sickle cell crisis. In addition we found significantly lower numbers of circulating platelet-MP in steady state and crisis. However, we also demonstrated that in crisis, a significantly higher percentage of MP express markers of endothelial and red cell origin. In addition we found composite hybrid MP expressing markers of more than one cell type,

probably brought about by severe vascular stress and close contact of various circulating cell types with vascular endothelium. We also found that vascular damage is evident in children with SCA during chest crises.
