Mehrere klinische Studien zeigen, dass Nahrungsvielfalt die Gesundheit über zahlreiche Wege fördert. Masset et al. (Massett 2015) haben gezeigt, dass eine höhere Nahrungsvielfalt mit einer 39%igen Reduktion koronarer Herzerkrankungen und einer 26%igen Reduktion der allgemeinen Mortalität einherging. Eine weitere Studie hat gezeigt, dass eine höhere Nahrungsvielfalt über einen sechswöchigen Zeitraum Gewichtszunahme signifikant verhindert (Ajdani 2013). Eines der größten Probleme der heutigen Zeit ist die Zunahme der Menschen, die an Adipositas und metabolischem Syndrom leiden. Vadiveloo (Vadiveloo 2015) hat kürzlich gezeigt, dass eine Diät mit hoher Diversität (gemessen mit dem US Healthy Food Diversity Index) mit einem um 21 % verringerten Risiko für die Entwicklung des metabolischen Syndroms assoziiert ist. Eine erhöhte Vielfalt beim Konsum von Obst und Gemüse zeigte keinen Zusammenhang mit einem reduzierten Risiko für Enddarmkrebs, obwohl dieses mit einer höheren Aufnahme von Obst und Gemüse assoziiert war (Leenders 2016). Eine erhöhte Nahrungsdiversität als Intervention zur Prävention der Sterblichkeit durch Herz-Kreislauf-Erkrankungen erwies sich als wirksamer als die Eliminierung ungesunder Nahrungsmittel (Michel 2002).

Ketosis was critical to human’s evolutionary development to provide energy while sparing muscle
mass [64]. The survival benefit is obvious: an average weight starving adult in ketosis survives for
about 2 months; while an average weight starving adult not in ketosis survives for 2– 3 weeks [3].
Moreover, prolonged low levels of insulin increase the amounts of the enzymes
required for ketone bodies synthesis (e.g., thiolase, HMG-CoA synthase, HMGCoA lyase, and β-
hydroxybutyrate dehydrogenase), and the enzymes required for ketone bodies utilization (e.g., β-
ketoacyl-CoA transferase, thiolase, and β-hydroxybutyrate dehydrogenase). Simultaneously, the
liver is also producing Acetyl-CoA from the free fatty acids liberated from triglycerides in adipose
tissue through β-oxidation. In the liver, the generated Acetyl-CoA is not being recycled through
the Krebs Cycle because the Krebs Cycle intermediates (mainly oxaloacetate) have been depleted
to feed the gluconeogenesis pathway. The resulting accumulation of Acetyl-CoA which is the
substrate for ketone body production and the increase in the amounts of the enzymes required for
ketone bodies synthesis induces ketosis. In ketosis, the liver synthesizes ketone bodies such as β-
hydroxybutyrate acid (BHB), Acetoacetate, and Acetone—which are released into the
bloodstream. In the metabolically active tissues, such as muscle or brain, ketone bodies are
converted to Acetyl-CoA which is used as metabolites in these tissues. BHB is the main ketone
body and is a fatty acid-like molecule. In humans, basal serum levels of BHB are in the low
micromolar range, but begin to rise to a few hundred micromolar after 12–16 hours of fasting,
reaching 1–2 mM after 2 days of fasting, and 6–8 mM with prolonged starvation [64]. Similar 1–2
mM levels of βOHB can be reached after 90 minutes of intense exercise [65]. Consistent levels
above 2 mM are also reached with a ketogenic diet [66].
