Bei der Behandlung des traumatologisch-bedingten Herzkreislaufstillstands soll beachtet werden, dass dieser eine andere Pathophysiologie als der nichttraumatologisch-bedingte Herzkreislaufstillstand hat und sich das Vorgehen daher grundlegend unterscheidet (A).

Bei (vermuteter) nicht-traumatischer Ursache eines Herzkreislaufstillstandes bei Traumapatienten soll nach den entsprechenden europäischen Reanimations-leitlinien vorgegangen werden (GPP).

Wird ein Spannungspneumothorax vermutet, soll bei Patienten mit Trauma-bedingtem Herzkreislauf-stillstand eine beidseitige Entlastung mittels Minithorakotomie vorgenommen werden (A).

Je nach Verletzungslokalisation kann als Zugang eine anterolaterale Thorakotomie oder eine Sternotomie gewählt werden. Bei unklarer Verletzungslokalisation kann der Clamshell-Zugang gewählt werden (0).

Access time was 2.39 min [1.21–2.76] for LAT and

2.33 min [1.58–4.86] for CI (p = 0.34).

Control time was 4.16 min [2.32–5.49] for LAT and

1.85 min [1.38–2.23] for CI (p = 0.018).

We report a case of on scene thoracotomy performed by an

anaesthesist on a patient in cardiac arrest following a stab

wound to the chest. The patient made a good recovery and

was discharged from the hospital within 2 weeks.

Thirty-nine prehospital thoracotomies were performed.

Four (10%) patients survived, one with long-term

disability.

Factors associated with survival were stab wound, single

cardiac wound, cardiac tamponade, and loss of pulse in the

presence of an experienced prehospital doctor.

Let’s face it: enormous efforts have been undertaken for decades to

educate, train and perform high quality resuscitation in patients with

out-of-hospital cardiac arrest even though the majority are in

asystole and have dismally low survival rates. Now the London HEMS

group have initiated a new and radical approach by performing

standardised resuscitative thoracotomy in the field. And they have

been successful – achieving survival rates of 18% in patients with

cardiac arrest following penetrating chest injuries.
