Sensibilità e specificità di alcuni strumenti epidemiologici, valutati usando come riferimento la diagnosi clinica di asma corrente (gold standard) [de Marco R et al, Eur Respir J, 1998; 11: 599-605]

“Alle domande sui sintomi respiratori negli ultimi 12 mesi risponde

NEGATIVAMENTE il 91.2-99.7% dei NON ASMATICI (specificità)

POSITIVAMENTE tra il 32 e il 68% degli ASMATICI (sensibilità).

La domanda migliore, con un’ottima specificità (97.5%) e una sensibilità non troppo bassa (67.6%) sembra essere la seguente (ever asthma):

“Nel corso della Sua vita ha mai avuto l’asma?”

La combinazione di attacchi d’asma e di asma corrente (asma corrente auto-riportata) non consente di migliorare la sensibilità (<37.1%), rispetto alla domanda ever asthma; anche la combinazione dei sintomi nell’anno precedente (sibili o mancanza di respiro o attacchi d’asma o farmaci antiasmatici) presenta una sensibilità discreta (82.9%) a spese di una riduzione della specificità (86.7%).

Abbinando ai sintomi nell’anno precedente, l’iperreattività bronchiale e i risultati dei test allergologici (IgE specifiche o prick test) si ottiene una sensibilità del 71.4% e una specificità del 95.2% rispetto alla diagnosi clinica.

In Italia le malattie dell’apparato respiratorio rappresentano la TERZA causa di morte, dopo le malattie del sistema circolatorio e i tumori.

Nel 2002 i decessi per malattie respiratorie (codici ICD IX 460-519) sono stati 35 736, pari al 6.4% del totale (555 667).

Questa percentuale è leggermente maggiore nei maschi (7.4 % = 20 488 / 276 315) e leggermente inferiore nelle donne (5.5% = 15 248 / 279 352).

RINITE ALLERGICA E ASMA

Diversi studi hanno dimostrato che

• la coesistenza di asma e rinite allergica è frequente, • la rinite allergica solitamente precede l’asma,• la rinite allergica è un fattore di rischio per l’asma.

La prevalenza di rinite allergica in soggetti asmatici in tali studi varia dall’80% al 90%.Viceversa, l’asma è più frequente in soggetti con rinite allergica che in soggetti senza rinite: odds ratio 6.63; 95% CI, 5.44-8.08.

CONCLUSIONI1)Le malattie respiratorie rappresentano attualmente la

TERZA causa di morte in Italia, e il loro peso non sembra destinato a scendere a causa dell’invecchiamento della popolazione.

2)L’asma corrente colpisce circa il 5% negli adulti e circa l’11% dei bambini, con grande variabilità a livello internazionale. La rinite allergica e la bronchite cronica affliggono rispettivamente il 19% e il 12% dei giovani adulti.

3)Mentre la patologia asmatica sembra avere raggiunto un plateau negli ultimi anni, la rinite allergica continua ad essere in aumento.

4)Agli inizi degli anni Novanta un terzo degli asmatici non erano consapevoli di esserlo.

5)La mortalità è notevolmente aumentata nei soggetti che soffrono di BroncoPneumopatia Cronica Ostruttiva

Although no cure for asthma has yet been found it is reasonable to expect that in most patients control of the disease can and should be achieved and maintained.

Control of asthma is defined as:

5) Nonostante l’aumento del consumo di farmaci antiasmatici (in particolare ICS) l’asma continua ad essere non controllata in Italia. Tra gli asmatici persistenti (55% di tutti gli asmatici) solo 2 su 10 utilizzano un trattamento adeguato alle linee guida. Le principali ragioni dello scarso controllo della malattia sembrano essere:

La scarsa regolarità nell’assunzione dei farmaci (il 67% dei pazienti con indicazione non li assume regolarmente)L’utilizzo di una combinazione terapeutica non adeguata alla propria severità (48% dei pazienti)

6) Di conseguenza, le malattie respiratorie comportano una non-trascurabile limitazione delle attività quotidiane.

Per identificare i casi di rinite allergica è stata utilizzata la seguente domanda [Burney et al, 1994; de Marco et al, 1998; Olivieri et al, 2002]:

“Ha raffreddori allergici compreso il ‘raffreddore da fieno’?”

Sono state altresì considerate le seguenti domande sui sintomi nasali:

52.1-6 “Quando viene in contatto con animali (es. cani, gatti, cavalli), con piume (compresi cuscini, piumoni, trapunte) o con una parte polverosa della casa, le capita mai di:

52.5 avere il naso che cola o chiuso o di incominciare a starnutire?”

53.1-6 “Quando si trova vicino a piante, erbe o fiori o quando c’è molto polline in giro, le capita mai di:

53.5 avere il naso che cola o chiuso o di incominciare a starnutire?”

Rinite allergica

Definition of Atopy

At the skin prick test “a mean wheal diameter of 3 mm or more was taken to be a positive reaction”.

“A subject was considered to be atopicif positive to one or more of the tested allergens at the skin prick test orwhen at least one specific IgE measurement was 0.35 kU/l (class 1)”

[Olivieri et al, 2002]

Accordingly to the GOLD guidelines [Pauwels et al, 2001] the presence of chronic cough and sputum production before airflow obstruction offers a unique opportunity to identify subjects at risk for COPD (GOLD stage 0) for an early intervention.

The following question, administered by a screening questionnaire ‘99, was used to identify chronic cough and phlegm in a screening questionnaire [Cerveri et al, 2003]:

“Have you had cough and phlegm on most days for as much as (a minimum of) three months per year and for at least two successive years?”

Q8. “Ha avuto tosse e catarro per la maggior parte dei giorni per almeno 3 mesi all’anno e da almeno 2 anni consecutivi?”

Chronic Obstructive Pulmonary Disease (COPD):Screening questionnaire

About 30% of subjects with subjects identified by this question were non-smokers. In comparison with current smokers, they comprised a higher proportion of females (58.9% vs 47.4%) and they reported more frequently asthma (26.4% vs 15.1%) and nasal allergies (44.5% vs 22.1%).

“The overall prevalence of subjects with chronic cough and phlegm was 11.9 % (95% CI: 11.4-12.4%)”. Of these 2.2% (2.0-2.4%) reported also coexisting asthma.

Hence, 18.5% of those subjects self-reporting chronic cough and phlegm also declare to suffer from asthma.

However, the Authors avoided the term COPD and rather used the phrase “chronic cough and phlegm”.

“With respect to clinicians’ diagnosis of current asthma, the specificity of the single questions pertaining to the last 12 months ranged from 91.2% to 99.7%, while the sensitivity was especially low, particularly for shortness of breath and asthma attack.

The single question maximizing the sum of sensitivity and specificity was that on asthma in the lifetime (ever-asthma), which had a sensitivity of 67.6% and a specificity of 97.5%.

The combination defined as self-reported current asthma [asthma attacks + medicines for asthma] did not improve the sensitivity, with respect to the single question on ever asthma, while the combination named past year symptoms [wheeze + shortness of breath + asthma attacks + medicines for asthma] achieved a good sensitivity (82.9%) at the expense of a substantial drop in specificity.

The use of past year symptoms associated with BHR only slightly increased the sensitivity. A noticeable improvement in sensitivity was obtained when adding the results of allergological tests: in this case the sensitivity was 71.4% and the specificity 95.2%.”
