Einleitung
cerv-X ist ein Cage zur zervikalen inter-somatischen Fusion bestehend aus rönt-genstrahldurchlässigem PEEK Optima® LT1.
Der cerv-X wird über einen ventralen Zugang in den Zwischenwirbelraum einge bracht. Mit Hilfe des Cages ist eine Über brückung des Zwischen-wirbel raumes möglich und damit die Wiederherstellung der Zwischen wirbel-höhe und die Re kon struktion des Wirbel säulen profils. Eine zusätzliche Stabilisierung mit einer zervikalen Platte, z.B. mambo oder osmium wird dringend empfohlen.
Um die korrekte Position des Implantates im Zwischenwirbelraum zu bestimmen, enthält der cerv-X-Cage Röntgenmarker.
Die Implantate werden einzeln steril ver packt geliefert. Zum Einsetzen der Im plan tate dürfen ausschließlich die dafür vorgesehenen Originalinstrumente des Implantatsystems verwendet wer-den.
Die vorliegende OP-Technik beschreibt das Implantat und die Instrumente sowie die Arbeitsschritte für die Anwendung des cerv-X-Cages. Sie ist als alleini-ge Grundlage für die erfolgreiche An wendung des cerv-X-Cages nicht aus reichend. Es wird empfohlen, die Operations technik bei einem erfahrenen Operateur zu erlernen.
Bitte beachten Sie die Gebrauchs-anweisung für cerv-X Implantate und die Wiederaufbereitungsanweisung für Instrumente.

Introduction
cerv-X is a cage for cervical inter body fusion, consisting of radiolucent PEEK Optima® LT1.
The cerv-X is inserted into the interver-tebral space using an anterior approach. With the cage, it is possible to bridge the intervertebral space, thereby recon-structing the spinal profile and restoring the intervertebral height. An addition-al stabilization with a cervical plate is re commended, e.g. mambo or osmium.
For establishing the correct position of the implant in the intervertebral space, the cerv-X cage contains radiographic markers.
The implants are single sterile packaged. To insert the implants, only the original instruments from the implant system designated for this purpose may be used.
The present surgical technique describes the implant and the instruments as well as the procedures for using the cerv-X cage. It is not sufficient as the sole basis for the successful application of the cerv-X cage. It is recommended to study and learn the operating technique with and from an experienced surgeon.

Indikationen und Kontraindikationen
Indikationen
cerv-X ist ein Implantat zur Abstützung des Zwischenwirbelraumes im Bereich der menschlichen zervika-len Wirbelsäule von C3 bis C7 mit dem Ziel der Unterstützung der Spondylodese. Die Implantate können zugleich zur Wiederherstellung des Wirbelsäulenprofils dienen. Eine zusätzliche Stabilisierung wird dringend empfohlen.
Fusionsoperationen sind z.B. indiziert für: � Degenerative Bandscheibenerkrankung � Spondylolisthese und Retrolisthese Grad 1 � Degenerative Skoliose � Revisionsoperation � Pseudarthrose
Kontraindikationen
� Patienten mit Fieber oder Leukozytose � Patienten mit Wirbelsäule assoziierten Infektionen (z. B. Spondylodiszitis)
� Patienten mit nachgewiesener Materialallergie oder Neigung zu Fremdkörperreaktionen
� Patienten ohne adäquate Compliance, die aufgrund ihres geistigen bzw. neurologischen Zustands nicht willens oder fähig sind, sich an die Nachsorgeanweisungen zu halten
� Patienten mit einem ungünstigen medizinischen oder psy-chologischen Allgemeinzustand, der durch den Eingriff weiter verschlechtert werden könnte; bei diesen Patienten ist eine sorgfältige Abwägung durch den behandelnden Chirurg/Arzt vorzunehmen
� Patienten mit unzureichender Knochenqualität oder -quan-tität, z.B. schwere Osteoporose, Osteopenie, Osteomyelitis
� Schwangerschaft
Indications and contraindications
Indications
cerv-X is an implant for supporting the intervertebral space in the cervical spine in humans from C3 to C7 with the purpose of providing support for spondylodesis. The implants may also be used for restoring the spinal profile. An additional stabilization is strongly recommended.
Indications for fusion operations are for example: � Degenerative disk disease � Spondylolisthesis and retrolisthesis grade 1 � Degenerative scoliosis � Revision surgery � Pseudarthrosis
Contraindications
� Patients with fever or leukocytosis � Patients with spine associated infections (e.g. spondylodis-citis)
� Patients with proven material allergy or tendency to react to foreign bodies
� Patients without adequate compliance who are not able due to their mental and/or neurological state or capable of adhering to the aftercare instructions
� Patients with a poor general medical or psychological condition which could be further exacerbated by the inter-vention; in these patients the attending surgeon/physician must carefully weigh the risks and benefits
� Patients with insufficient bone quality or quantity, e.g. with severe osteoporosis, osteopenia, osteomyelitis
� Pregnancy

Operationstechnik
Diskektomie
Der Zwischenwirbelraum wird dargestellt und das Band-schei ben material, z. B. mit Rongeuren, entfernt. Die End-platten werden präpariert. Im Anschluss wird das Segment mit einem Spreizer für die zer-vikale Wirbelsäule distrahiert.
Bestimmung der Implantatgröße
Mit Hilfe des Probeimplantats wird die benötigte Implantat-größe bestimmt. Zur Auswahl stehen gewölbte Cages und Cages mit 5° Winkelung in 2 Größen mit Höhen von 5 bis 8 mm. Für jedes Implantat steht ein passendes Probeimplantat zur Verfügung. Das Probeimplantat (CS 8210-XX, CS 8212-XX, CS 8214-XX bzw. CS 8216-XX) wird auf den Halter (CS 8210) aufge-schraubt. Die Probeimplantate sind farbkodiert. Die ent-sprechende Farbkodierung findet sich ebenfalls auf der Verpackung der Implantate.
Patientenlagerung und Zugang
Der Patient wird auf den Rücken gelagert. Die Lagerung, die Inzision und der Zugang erfolgen in typischer Weise zur Durchführung einer ven-tralen zervikalen Diskektomie.
Surgical technique
Diskectomy
The intervertebral space is exposed and disk material is removed with rongeurs for example. The end plates are prepared. Afterwards the seg-ment is distracted by a retrac-tor for the cervical spine.
Determining the implant size
By means of the trial implants the implant size required is determined. You can choose between wave cages and cages with angle 5° with 2 sizes and heights from 5 to 8 mm. For each implant a corresponding trial implant is available. The trial implant (CS 8210-XX, CS 8212-XX, CS 8214-XX or CS 8216-XX) is connected to the holder (CS 8210) by screwing in. The trial implants are color-coded. The packaging of the implants is also marked with the corre-sponding color-coding.
Patient positioning and approach
The patient is placed in a supine position. The positioning, the incision and the approach are done in the usual manner for anterior cervical diskectomy.

Der Fixierstab (CS 8220-02) wird in das Einsetzinstrument eingeführt. Es steht wahlwei-se ein Einsetzinstrument ohne Tiefenanschlag (CS 8220-01) und mit Tiefenanschlag (CS 8220-03) zur Verfügung. Der Cage wird auf das Instrument aufgesetzt und festge-schraubt.
Befüllen des Implantats
Das Implantat kann mit Knochen material oder Kno-chen ersatz material befüllt werden. Zum Befüllen und Ver dich ten des Materials ste-hen ein Befüll block (CS 8223) und ein Spongiosa stößel (CS 8222) zur Verfügung.
Surgical technique
Assembly of implant and inserter
The locking rod (CS 8220-02) is inserted into the inserter. The inserter is available with-out depth stop (CS 8220-01) and with depth stop (CS 8220-03). The inserter is positioned onto the cage by screwing in.
Implant filling
The implant can be filled with bone graft or bone substitute. A filling aid (CS 8223) and a tamper (CS 8222) are avail-able for filling and packing the bone material.
