Argumente für die allogene SZT
• Es gibt einen potenten Graft – versus –Myeloma – Effekt ( 60% RR, 30% CR) nach Spenderlymphozytengabe
• Nur nach allogener SZT erreicht ein signifikanter Anteil der Patienten eine molekulare Remission (30 – 50%)
• Nur die Überlebenskurven nach allogenerSZT weisen ein Plateau auf ( 30 – 40% )
Probleme der allogenen SZT
• Höheres Durchschnittsalter der Patienten mit erhöhter Organtoxizität
• Höheres GvHD – Risiko wegen Alter• Schwerer Immundefekt schon vor SZT
(Krankheit, Therapie)• Folge dieser Faktoren : Hohe TRM, 1983 –
1993 50%, 1993 – 1998 30%• Mäßige CR – Rate 50 – 60%• Hohe Rezidivrate 50%
