Ruggenmergstimulatie voor chronische pijn moet actueel kunnen uitgevoerd worden bij actueel bestaande of vroegere aantasting van weefselstructuren resulterend in:
- neuropathische pijn
- ischemische pijn
Ruggenmergstimulatie voor nociceptievepijn is niet geïndiceerd.
Volgens de nomenclatuur zoals gepubliceerd in het Belgisch Staatsblad kan ruggenmergstimulatie uitgevoerd worden bij 3 indicaties:
1. Neurogene pijn 2. Trombo-angitis obliterans (ziekte van Buerger) 3. Perifeer kritiek niet-opereerbaar ischemisch vaatlijden

Indien het gaat om een gemengd pijnsyndroom dan kan ruggenmergstimulatie aangewend worden voor neuropathische pijn en/of ischemische pijn. Indien het gaat om een gemengd pijnsyndroom omwille van de aanwezigheid van een nociceptief pijnsyndroom dan dient de component neuropathische pijn of ischemische pijn de dominante te zijn in het klinisch verhaal en qua intensiteit van de pijn. Onder een gemengd pijnsyndroom word t ook sympathisch gemedieerde pijn beschouwd zoals bij het Complex Regionaal Pijnsyndroom.
Andere indicaties neurogene pijn:
Goede analgesie pijn na CRPSpartiële plexuslesiesperifeer neurotrauma
Slechte respons op SCS:complete plexuslesiescomplete ruggenmerglesienociceptieve pijn
Meest voorkomende indicatie in de pijnkliniek: Failed Back Surgery Syndrome = FBSS
Beste analgesie: radiculaire klachten > LBPunilaterale uitstralingallodynie – hyperalgesie
Minder goed te beheersen met SCS:rugpijn zelfbilaterale uitstralingmechanische pijntypes

1. Multidisciplinair overleg neurochirurgie en pijncentrum: • Pijntype komt in aanmerking• Andere therapeutische opties uitgeput• Meest geschikte type elektrode en plaatsing: chirurgisch of percutaan
2. Intake psychiater: geen contra-indicatie3. Intake psycholoog:
• Pre-stimulatie meting van o.m. pijn, functionele impact en analgeticagebruik
4. Implantatie van de proefelektrode met uitwendige extensie ziekenhuisopname 1 nacht - enkele dagen
5. Proefperiode: 4 weken waarvan minimum 3 weken thuis, Dagelijkse verzorging en controle door thuisverpleging
6. Psycholoog na 4 weken: herevaluatie pijnparameters- Verwijderen proefelektrode+ Implantatie IPG

• Dislocatie en malfunctie van de onderdelen• Stimulatie dorsale wortel met motorisch antwoord Het toestel kan worden uitgeschakeld door de patiënt
• Infectie• Perforatie van de dura en postdurale punctie hoofdpijn• Epidurale bloeding en hematoom
• Pijn over de onderhuids geïmplanteerde batterij: frequent
Frequente bezoeken PCT blijven vaak noodzakelijk, ook bij gunstig effect SCS
