Fieber unklarer Genese

• Fieber ≥ 38,5°C (axillär) • Dauer >8 Tage • Unbekannte Ursache
• In der klinischen Praxis wird das Fieber unklarer Genese meist durch eine rektal gemessene Temperatur ≥ 38,5 oC mit einer Dauer von mehr als 8 Tagen definiert. In der Literatur findet sich eine variable Fieberdauer von 7 bis zu 21 Tagen als Einschlusskriterium; in allen Fälle konnte trotz einer ausführlichen Diagnostik keine Diagnose gestellt werden.

Hauptkriterien 1. Peritonitis (generalisiert) 2. Pleuritis (einseitig) oder Perikarditis 3. Monarthritis (Hüfte, Knie, OSG) 4. Fieber (ohne zusätzliche Symptome) 5. Gutes Ansprechen auf Colchicin Zusatzkriterien 1. Positive Familienanamnese bezüglich FMF 2. Passender ethnischer Ursprung 3. Alter <20 Jahre zu Krankheitsbeginn 4. Ausprägung, die Bettruhe erfordert 5. Spontane Remissionen 6. Symptomfreie Intervalle 7. Vorübergehende inflammatorische Reaktion (BSG, SAA oder Fibrinogen) 8. Episodische Proteinurie oder Hämaturie 9. Unergiebige Laparotomie oder Entfernung eines unauffälligen Appendix 10. Konsanguinität der Eltern

• Fieber 100% • Arthritis 100% • Exanthem 90% • Hepato-Splenomegalie 70-80% • Karditis 60% • Lymphadenopathie 55% • Pleuritis 15% • Iridozyklitis 2% • Makrophagenaktivierung • Langzeitfolgen (Minderwuchs, Arthrose, Amyloidose)
