Einleitung

Das uDuro™ kyphoplasty system ist ein Instrumentarium zur Aufrichtung einer Wirbel-Kompressionsfraktur. Nach erfolgter Aufrichtung wird zur Stabilisierung und Fixierung Knochenzement in das Cavum injiziert.

Mit uDuro™ bietet ulrich medical® ein System zur Behandlung von Wirbelkörperfrakturen an. Es werden zwei Systeme mit unterschiedlichen Ballonkathetern angeboten:uDuro™ kyphoplasty system – 15 mm CS 6601-15 beinhal-tet zwei Ballonkatheter mit je 15 mm Länge.uDuro™ kyphoplasty system – 20 mm CS 6601-20 beinhal-tet zwei Ballonkatheter mit je 20 mm Länge.

Für das minimal-invasive Verfahren stehen dem Operateur einzeln steril verpackte Instrumente zum Einmal-Gebrauch zur Verfügung. Die ergonomisch geformten Griffe ermöglichen eine leichte Manövrierbarkeit und garantieren dem Anwender ein hohes Maß an Komfort, Zuverlässigkeit und Sicherheit. Es dürfen ausschließlich nur die Originalinstrumente des uDuro™ Systems verwendet werden.

Die vorliegende OP-Technik beschreibt die Instrumente sowie die Arbeitsschritte für die Anwendung des uDuro™ Systems. Die OP-Technik ist als alleinige Grundlage für die erfolgreiche Anwendung des Kyphoplastie Systems uDuro™ nicht ausrei-chend. Es wird empfohlen, die Operationstechnik bei einem erfahrenen Operateur zu erlernen.

Indikationen

� Schmerzhafte osteoporotische BWS- und LWS-Frakturen � Schmerzhafte traumatische Frakturen � Schmerzhafte Osteolysen/Metastasen

Kontraindikationen

Absolut � Patienten mit nachgewiesener Materialallergie oder Neigung zu Fremdkörperreaktionen

� Patienten mit nachgewiesener Kontrastmittelallergie � Osteomyelitis � Hämorrhagische Diathese � Vertebra plana

Relativ � Patienten mit einem ungünstigen medizinischen oder psychologischen Allgemeinzustand, der durch den Eingriff weiter verschlechtert werden könnte; bei diesen Patienten ist eine sorgfältige Abwägung durch den behandelnden Chirurg/Arzt vorzunehmen

� Unzureichende Ausschöpfung konservativer Maßnahmen � Gerinnungsstörung � Schwangerschaft

Introduction

The uDuro kyphoplasty system consists of instruments for repositioning a vertebral compression fracture. After success-ful repositioning, bone cement is injected into the cavum for stabilization and fixation.

With uDuro™, ulrich medical® offers a system for treating vertebral body fractures. Two systems with different balloon catheters are available:uDuro™ kyphoplasty system – 15 mm CS 6601-15 includes two balloon catheters, each 15 mm in length.uDuro™ kyphoplasty system – 20 mm CS 6601-20 includes two balloon catheters, each 20 mm in length.

For minimally invasive procedures, sterile, individually pack-aged instruments are available to the surgeon for single use. The ergonomically shaped handles allow easy maneuverability and ensure a high degree of comfort, reliability and safety for the user. Only genuine instruments of the uDuro™ system may be used. The present surgical technique describes the instruments as well as the procedures for using the uDuro™ system. It is not sufficient as the sole basis for the successful application of the uDuro™ system. It is recommended to study and learn the operating technique with and from an experienced surgon.

� Painful opsteoporotic thoracic and lumbar vertebral frac-tures

� Painful traumatic fractures � Painful osteolyses / metastases

Contraindications

Absolute � Patients with a documented history of material allergy or who tend to react to foreign bodies

� Patients with documented contrast agent allergies � Osteomyelitis � Hemorrhagic diathesis � Vertebra plana

Relative � Patients whose general medical or psychological condition is unfavorable for or could be worsened by the proce-dure; careful consideration is required on the part of the treating physician/surgeon for these patients

� Underutilization of conservative measures � Coagulopathy � Pregnancy

OperationstechnikSurgical technique

Eintrittspunkt für die Kno-chenpunktionsnadel (CS 6601-01) auf der Haut des Patienten markieren.

Transpedikulärer Zugang

Transpedikulärer Zugang mit der Knochenpunktionsnadel (CS 6601-01) bis 5 mm in den Wirbelkörper.

Entfernung des Trokars

Trokar aus der Knochenpunk-tionsnadel (CS 6601-01) zie-hen.

Einbringung des Führungsdrahtes

Führungsdraht (CS 6601-02) bis zum letzten Viertel in den Wirbelkörper einbringen.

Mark the point of entry for the bone puncture needle (CS 6601-01) on the skin of the patient.

Transpedicular access

Transpedicular access with the bone puncture needle (CS 6601-01) up to 5 mm into the vertebral body.

Removing the trocar

Pull the trocar out of the bone puncture needle (CS 6601-01).

Inserting the guide wire

Insert the guide wire (CS 6601-02) into the vertebral body up to the last quarter.

Removing the bone puncture needle

Remove the bone puncture needle (CS 6601-01) from the vertebral body, the guide wire (CS 6601-02) remains in the vertebral body.

Insert the cannula (CS 6601-03) over the guide wire (CS 6601-02) into the vertebral body.

Removing the guide wire

Remove the guide wire (CS 6601-02), only the cannula (CS 6601-03) remains in the vertebral body.

Entfernung des Knochenbohrers

Der Knochenbohrer (CS 6601-04) wird entfernt.

Glättung/Entgratung des Cavums

Der Knochenzementapplikator (CS 6601-07) wird mehrmals durch die Kanüle (CS 6601-03) hin- und her bewegt, um ein Platzen des Ballons durch scharfe Knochensplitter zu vermeiden. Danach muss der Knochenzementapplikator (CS 6601-07) wieder entfernt werden.

Vorbereitung des Ballonexpanders

Der Verschluss des Ballonex-panders (CS 6601-06) wird auf „unlock“ gestellt.

Removing the bone drill

Remove the bone drill (CS 6601-04).

Smoothing/deburring the cavum

Move the bone cement appli-cator (CS 6601-07) back and forth through the cannula (CS 6601-03) several times to pre-vent the balloon from bursting due to sharp bone fragments. The bone cement applicator (CS 6601-07) must then be removed.

Preparing the balloon expander

Move the seal of the balloon expander (CS 6601-06) to the “unlock” position.

Auffüllung des Zylinders mit Kontrastmittel

Durch Zug am Griff des Ballonexpanders wird der Zylinder mit ca. 20 cm3 Kontrastmittel gefüllt.

Anschließend wird der Verschluss auf „lock“ gestellt.

Entfernung der Luft aus dem Zylinder

Der Verschluss des Ballonex-panders (CS 6601-06) bleibt verriegelt.

Durch Drehung des Griffs nach rechts bis kurz vor die „0“-Markierung, wird Luft aus dem Zylinder entzogen.

Verbindung mit Ballonkatheter

Filling the cylinder with contrast agent

Pull the handle of the balloon expander to fill the cylinder with approx. 20 cm3 of con-trast agent.

Then move the seal to the “lock” position.

Removing air from the cylinder

The seal of the balloon ex-pander (CS 6601-06) remains locked.

Turn the handle to the right just before the “0” marking to remove air from the cylinder.

Entfernung der Restluft aus dem System

Die Abdeckung des Ballon-katheters (CS 6601-05-15 oder CS 6601-05-20) wird entfernt. Das 3-Wege Ventil zeigt mit „off“ zum freien Anschluss.

Um die restliche Luft zu ent-fernen, wird der Griff des Ballonexpanders (CS 6601-06) nach rechts gedreht, dabei füllt sich der Ballon.

Um die Luft wieder aus dem Ballon des Ballonkatheters (CS 6601-05-15 oder CS 6601-05-20) zu entnehmen, wird der Verriegelungsmechanis-mus auf „unlock“ gestellt. Der Griff des Ballonkatheters wird aufgezogen und der Verriegel-ungsmechanismus auf „lock“ gestellt.

Removing residual air from the system

Remove the cover of the balloon catheter (CS 6601-05-15 or CS 6601-05-20). The 3-way valve points to the open connection with “off”.

To remove the remaining air, turn the handle of the balloon expander (CS 6601-06) to the right. The balloon inflates.

To remove the air from the balloon of the balloon catheter (CS 6601-05-15 or CS 6601-05-20), move the locking mechanism to the “unlock” position. Pull the handle of the balloon catheter and move the locking mecha-nism to the “lock” position.

Einsetzen des Ballonkatheters

Der Ballonkatheter (CS 6601-05-15 oder CS 6601-05-20) wird durch die Kanüle (CS 6601-03) in den Wirbelkörper eingeführt. Es muss sicherge-stellt werden, dass der Ballon vollständig aus der Kanüle austritt.

Die Beschriftung „off“ des Ventils Richtung Ballon dre-hen.

Durch Drehung des Griffs nach rechts entweicht die restliche Luft aus dem System.

Die Beschriftung „off“ des Ventils in Richtung freien Anschluss drehen.

Inserting the balloon catheter

Insert the balloon catheter (CS 6601-05-15 or CS 6601-05-20) through the cannula (CS 6601-03) into the vertebral body. Ensure that the balloon exits the cannula completely.

Turn the “off” label on the valve towards the balloon.

By turning the handle to the right, the remaining air escapes from the system.

Turn the “off” label on the valve towards the open con-nection.

Aufpumpen des Ballons mit Kontrastmittel

Der Griff des Ballonexpanders (CS 6601-06) wird nach rechts gedreht und der Ballon wird mit Kontrastmittel aufge-pumpt.

Entleeren des Ballons

Verschluss auf “unlock” stel-len.

Den Griff des Ballonexpanders (CS 6601-06) nach unten ziehen, um das Kontrastmittel aus dem Ballon zu entziehen.

Den Verriegelungsmechanis-mus anschließend auf „lock“stellen.

Den Ballonkatheter (CS 6601-05-15 oder CS 6601-05-20) aus der Kanüle (CS 6601-03) herausziehen.

Inflating the balloon with contrast agent

Turn the handle of the balloon expander (CS 6601-06) to the right to inflate the balloon with contrast agent.

Deflating the balloon

Move the seal to the “unlock” position.

Pull the handle of the balloon expander (CS 6601-06) down-ward to remove the contrast agent from the balloon.

Then move the locking mech-anism to the “lock” position.

Pull the balloon catheter (CS 6601-05-15 or CS 6601-05-20) out of the cannula (CS 6601-03).

Einsatz Knochen-zementapplikator

Der Knochenzementapplikator (CS 6601-07) wird durch die Kanüle (CS 6601-03) mit oder ohne Zement eingesetzt.

Befüllung mit Knochen zement

Mit dem Stempel des Knochenzementapplikators (CS 6601-07) wird der Knochenzement in den Hohlraum gedrückt.

Entfernung aller Instrumente

Inserting the bone cement applicator

Insert the bone cement appli-cator (CS 6601-07) through the cannula (CS 6601-03) with or without cement.

Filling with bone cement

The piston of the bone cement applicator (CS 6601-07) forces the bone cement into the cav-ity.
